Cetrorelix and Ganirelix

Cetrorelix (Cetrotide) and ganirelix (Antagon/Orgalutran) are real, FDA-approved GnRH antagonists used in IVF to prevent premature ovulation — grouped on one page because they're used identically in practice, though a real 2025 comparison found a meaningful clinical difference between them.

The short version

  • Both are GnRH antagonists — no flare, immediate LH/FSH suppression, used only during controlled ovarian stimulation to prevent a premature LH surge.
  • Ganirelix was approved first (29 July 1999) vs. cetrorelix (11 August 2000), but a 2025 real-world comparison found cetrorelix had better LH-surge control and a lower OHSS rate.
  • Live birth rates were statistically similar between the two in that comparison — the difference is in side-effect profile, not overall success.

Antagonists, used only in IVF

Cetrorelix and ganirelix are both heavily modified decapeptide GnRH antagonists, used during IVF/ICSI ovarian stimulation to prevent a premature LH surge that would trigger ovulation before egg retrieval. Unlike the GnRH agonists on this site (leuprolide, triptorelin, goserelin), which initially stimulate the pituitary receptor before eventually suppressing it, both of these directly and immediately block the receptor — there is no flare, and LH/FSH suppression is dose-dependent from the first injection. Both carry Pregnancy Category X labeling (not relevant beyond the stimulation window, since neither is used once pregnancy is achieved) and both have post-marketing reports of hypersensitivity reactions, including anaphylaxis, sometimes with the first dose.

The approvals and pivotal trials

Ganirelix (as Antagon) was the first GnRH antagonist approved in the US, on 29 July 1999, based on a randomized non-inferiority trial against a long buserelin-agonist protocol (European Orgalutran Study Group, Hum Reprod 2000, PMID 10875855) that found a materially lower rate of ovarian hyperstimulation syndrome (OHSS) with ganirelix (2.4%) than the agonist reference arm (5.9%), with comparable pregnancy rates. Cetrorelix (as Cetrotide) followed on 11 August 2000. The Antagon brand has since been discontinued in the US, though generic ganirelix acetate remains available; Cetrotide remains branded, with at least one FDA-approved generic (Akorn).

The real difference between them

For years the two were treated as functionally interchangeable. A 2025 retrospective cohort study directly comparing them (Front Reprod Health 2025, PMID 40297130; n=2,365 cetrorelix vs. 7,059 ganirelix, propensity-matched) found real differences: ganirelix had a higher overall OHSS incidence than cetrorelix (1.1% vs. 0.4%, p=0.01), while cetrorelix showed better control of LH surges (LH ≥10 U/L in 4.9% vs. 7.6% of cycles, p<0.001). Live birth rates were statistically similar between the two (47.2% vs. 49.4%, p=0.074) — meaning the practical takeaway isn't "one works better," it's that cetrorelix may be the more conservative choice for patients at higher OHSS risk specifically.

Current status

Both remain in active clinical use for IVF, prescribed only through fertility clinics under refrigerated, controlled distribution — not a drug class with any meaningful presence among "research chemical" grey-market vendors, and we found no FDA warning letter naming either drug.
